DEA registrants can order Schedule I and II substances only through the Form 222. There are specific rules for using, signing and ordering these forms which extend to lost and unused forms as well.
The controlled substances regulations require that people who are registered to use Schedule I and II substances should order them by way of the DEA form 222. The form must be filled carefully with a pen, indelible pencil or typewriter. The lines are numbered and only one item can be filled on each line. The number of lines completed must be duly recorded in the space provided at the bottom of the form. Name and address of the supplier must be entered before signing and dating the form by the registrant or another person authorized to do so.
How to order DEA Form 222?
Any person applying for a DEA registration to use Schedule I or II substances can requisition the forms by so indicating on the application form or by contacting any Division Office or the Registration Section of the Administration. Subsequent requisitions should be made through the DEA Form 222a, which is mailed to a registrant approximately 30 days after each shipment of DEA Forms 222. Alternatively, they can contact the Division Office or the Registration Section.
Each requisition must show the name, address and registration number of the registrant along with the number of books of DEA Forms 222 required. It must be signed and datedby the registrant or another person authorized to do so.
The forms are issued in mailing envelopes containing either seven or fourteen forms. Based on the business activity of the registrant, a limit is imposed on the number of DEA Forms 222 that will be issued to him/her. Additional forms have to be specifically requested along with a reasonable need for the same.
Forms 222 are issued with the name, address and registration number of the registrant, the authorized activity, and schedules. Any errors therein must be corrected by returning them to the Registration Section with a notification of the error. The forms were issued in triplicate with interleaved carbon sheets earlier. In October 2019, the DEA amended to a single sheet format with a two-year transition period.
Lost or stolen forms
In case a registrant finds that a DEA Form 222 is missing, he/she must fill the next one and attach a statement stating the number and date of the missing form while indicating that the goods covered by the same were not received. Loss or theft of used/unused forms must be immediately reported to the local DEA Diversion Field Office with the serial number(s) of the same. A notification has to be sent immediately in case it is found or recovered later. If the supplier happens to locate the missing form, he/she must mark it as ‘Not Accepted’ and return it to the registrant.
Similarly, if the registrant closes the business or loses the registration, all unused forms should be promptly returned to the DEA Headquarters Registration Section.
